ST. HELENA PARISH HOSPITAL SERVICE DISTRICT NO. 1

REQUEST FOR PROPOSALS

ELECTRONIC HEALTH RECORD SYSTEM –

ST. HELENA PARISH HOSPITAL EMERGENCY DEPARTMENT

St. Helena Parish Hospital Service District No. 1 (the “HSD”) is soliciting sealed proposals from qualified hospital information technology vendors to acquire hardware, software, system components and services for a state-of-the-art electronic health record system for St. Helena Parish Hospital’s (the “Hospital”) emergency department that will seamlessly integrate with the Hospital’s existing day-to-day business and clinical systems in order for the Hospital to achieve meaningful use objectives and qualify for federal funding incentive payments.

The following criteria will be used to evaluate each firm submitting a proposal:

1. Professional Experience of the Firm

2. Completeness and Applicability of Proposed Solution

a. Integrated Solution

b. Meaningful Use

c. Infrastructure Environment

d. Implementation Plan

e. Development and Maintenance and Support Plan

f. Training Plan

g. Quality Assurance/Control

3. Cost of Solution

Complete details on the scope of the project, requested services and supporting information are set forth in the HSD’s competitive Request for Proposal packet (the “RFP”). Interested firms must submit one (1) signed original, ten (10) copies and one (1) electronic copy (on CD/DVD or thumb drive) of their proposal to the HSD, Attn: Naveed Awan, Chief Executive Officer, 16874 Highway 43, Greensburg, Louisiana 70441 before 10:00AM on August 31, 2012 (the “Deadline”). Proposals received after the Deadline will not be considered. Proposals received by the Deadline will be evaluated by the HSD in accordance with the selection and rating system set forth in the RFP at a meeting to be held at the Hospital, at 16874 Highway 43, Greensburg, Louisiana 70441, on September 10, 2012 at 4:30PM. The HSD reserves the right to reject any or all proposals.

The HSD is the recipient of, and intends to utilize, federal funds from the United States Department of Agriculture Rural Development (“USDA”) to acquire the system components requested in the RFP. Interested firms are required to comply with any and all state and federal laws and regulations (collectively “Legal Requirements”) governing HSD’s use of its funds and/or USDA funds for the acquisition of system components hereunder. Failure of the successful proposer to accept and comply with the Legal Requirements shall result in rejection of the proposal or termination of the contract.
